The $1,600 figure sometimes quoted includes lactation visits and community navigation; the standard prenatal/postpartum + birth package is $1,250. Doula services must be recommended by a physician or other licensed practitioner; there's no statewide standing recommendation (bills to create one were introduced in 2025-26). Coverage runs through 12 months postpartum. Managed care plans follow these rules but may add prior-authorization requirements. Rates come from MO HealthNet's 2026 Doula Provider Guide and Bulletin 48-51. The federally approved plan amendment (SPA MO-24-0008) is effective Oct. 1, 2024.
