How we built the tracker
The Mom Site built this tracker in October 2026 (research dated October 9, 2026) from primary sources for each state and DC: state Medicaid provider manuals and billing guides, provider bulletins and notices, fee schedules, administrative rules, state plan amendments, and managed care plan handbooks and policies where a state runs coverage through health plans. We downloaded each document and matched the supporting text before recording a value; anything we couldn't confirm is left blank rather than guessed.
Federal rules come from HRSA, the Code of Federal Regulations, the joint DOL/HHS/Treasury ACA FAQs and HealthCare.gov. Status: covered means Medicaid names lactation consultation or counseling as a separately payable service outside the hospital stay, available to members generally, whether the lactation consultant bills directly or a supervising enrolled provider bills for their services. Limited means lactation help exists only in a narrower form: inside bundled maternity, hospital or program payments; through general counseling or office visit codes with no lactation consultant pathway; only as classes; only in some managed care plans or as an extra plan benefit; or only for certain conditions, programs or provider settings.
Not-covered means the state's current fee schedule or policy lists the lactation-specific service as not covered and we found no other lactation pathway. Unverified means we couldn't confirm the state's rules from a primary source; the notes say what we checked. Lactation help given during a hospital stay, or as part of a routine prenatal, postpartum or well-child visit, isn't counted as a separate benefit. "IBCLCs can enroll on their own" is Yes when an IBCLC (or the state's certified or licensed lactation consultant) can enroll and bill Medicaid, or contract with its health plans, as their own provider without a separate clinical license; it's No when they must bill through a supervising provider or must also be licensed as, for example, a registered nurse, and blank when the sources don't say.
For comparison, each state record also keeps the state's answer to KFF's 2021 survey (coverage as of July 1, 2021), left blank for states that didn't respond. It wasn't used to set current status. Medicaid rules change often and individual health plans can add benefits, so readers should confirm with their state Medicaid agency or plan.
What to know nationally
KFF's last state survey (41 states and DC responding, coverage as of July 1, 2021) found 30 covered lactation consultation in the hospital, 27 in outpatient or clinic settings and 20 as home visits. Eight states (AZ, FL, LA, MS, ND, RI, TN, TX) reported no coverage of breastfeeding education or lactation consultation, and 15 reported covering every breastfeeding support KFF asked about. These are state self-reports, and nine states did not respond.

Medicaid often pays for lactation help inside other payments rather than as its own service. In KFF's 2021 survey, 23 states that covered hospital lactation consultation paid for it as part of the hospital DRG or global maternity fee and four paid separately; for outpatient lactation consultation, 13 states paid separately and 11 included it in an office visit. Some states also fold breastfeeding education into the global maternity fee. In this tracker, help that is only part of a hospital stay or a routine prenatal, postpartum or well-child visit isn't counted as a separate benefit.

Adults covered through the ACA Medicaid expansion get an Alternative Benefit Plan, which must include essential health benefits, and essential health benefits must include the preventive services in 45 CFR 147.130. KFF notes that states must cover breast pumps and lactation consultation for expansion enrollees under the ACA's preventive services requirement, and that non-expansion states have no federal requirement to cover breastfeeding services.

Most private health plans (job-based and Marketplace plans that aren't grandfathered) must cover lactation support with no copay, coinsurance or deductible. HRSA's Women's Preventive Services Guidelines call for comprehensive lactation support services, including consultation, counseling, education by clinicians and peer support services, and breastfeeding equipment and supplies, during pregnancy, around birth and after birth. Section 2713 of the Public Health Service Act and 45 CFR 147.130 make HRSA's women's guidelines a coverage requirement.

Coverage of lactation support and counseling extends for the duration of breastfeeding. Plans may use reasonable medical management to decide frequency, method, treatment or setting where the guidelines don't specify, but limiting lactation counseling to hospital stays is not a reasonable medical management technique.

Where a state doesn't license lactation consultants, plans must still cover lactation counseling, subject to reasonable medical management, when it's given by any provider acting within the scope of their license or certification under state law (for example, a registered nurse). Federal rules don't set how lactation consultants are paid; the Departments said reimbursement policy is outside the scope of the guidelines and regulations.

If a plan's network has no provider who can give lactation counseling, the plan must cover lactation counseling from an out-of-network provider with no cost sharing. Plans must also make a list of network lactation counseling providers available.

Grandfathered plans (generally individual policies bought on or before March 23, 2010, and job-based plans that haven't changed substantially since then) don't have to follow the preventive services requirement, including free lactation support. Some offer it anyway, so members should check their plan.

WIC offers breastfeeding support at no cost, including counseling and educational materials from trained staff. USDA's national breastfeeding peer counseling program trains peer counselors, recruited from WIC participants, to support WIC mothers in clinics, homes and hospitals.

TRICARE covers up to six individual outpatient breastfeeding counseling sessions per birth event from an authorized doctor, physician assistant, nurse practitioner, nurse midwife or registered nurse, in a clinic, provider office or outpatient hospital, or by virtual health. Non-medical lactation consultants and counselors are covered for TRICARE Prime, Prime Remote and Select members through the Childbirth and Breastfeeding Support Demonstration, which runs from January 1, 2022 to December 31, 2031.

Does Medicaid cover lactation consultants?
As of October 2026, 23 states and DC pay for lactation consultation as its own Medicaid service. 16 more cover it only in a narrower form: inside bundled maternity or hospital payments, as classes, through some health plans, or for certain conditions or settings. 4 don't cover it. We couldn't confirm 7; check with the state Medicaid agency or your plan.
Can an IBCLC bill Medicaid directly?
In 11 states and DC, an IBCLC or state-recognized lactation consultant can enroll and bill Medicaid, or contract with its health plans, on their own (in Louisiana, only through health plans that opt in). Elsewhere, a supervising enrolled provider bills for the visit, or the consultant also needs a clinical license such as an RN.
Is this the same as the breast pump benefit?
No. Breast pumps are covered as equipment, under separate rules. Each state's page shows both.
