Indiana Medicaid's June 2026 professional fee schedule lists S9443 (lactation class) as a non-covered service, and no lactation benefit or lactation consultant provider type appears in the IHCP provider reference modules. Breastfeeding evaluation and instruction are part of routine newborn well-child (EPSDT) visits. KFF's 2021 survey listed Indiana as covering lactation consultation in all settings, including separately paid outpatient visits. Health plan extra benefits were not checked.

The IHCP fee schedule lists manual (E0602) and electric (E0603) pumps as covered DME (purchase and rental) with no PA flag, and IHCP removed all PA for E0603 across fee-for-service and every managed care program effective August 26, 2022. Hospital-grade E0604 is listed as non-covered. A written order from an IHCP-enrolled physician, NP, CNS, CNM or PA is required for all DME. No frequency limit was found.
