Michigan details
Under MMP 24-40 and the January 2026 fee schedule, the maximum is 12 visits at $100 plus $1,500 for labor and delivery, or $2,700. SPA MI-22-0005 lists a 10/1/2022 effective date, but MMP 22-47 started coverage for dates of service on or after 1/1/2023. A statewide standing recommendation from the Chief Medical Executive meets the recommendation requirement. Visits are available during pregnancy and up to one year after delivery. Most members are in Medicaid Health Plans, and a 'Medicaid-enrolled' doula may not be in every plan's network, so moms should check their plan directory.
