The $100 incentive is excluded from the total. Coverage runs from pregnancy through 180 days postpartum, not 12 months. A licensed practitioner must recommend doula care before services start (DMAHS posts a recommendation form); there's no statewide standing order. Managed care members should use their plan's doula directory. The DMAHS page has a section for members and a section for doulas. We found no newer rate change.
