New Hampshire details
CMS approved New Hampshire's doula plan amendment (SPA 25-0015) on July 8, 2026, retroactive to Oct. 1, 2025, but we found no sign NH Medicaid is paying doulas yet. NH MMIS's covered-procedures and manually-priced fee schedules (as of 7/24/2026) do not list T1032 or T1033. The MMIS provider Messages & Announcements page (latest item 9/21/2026) has no doula notice, and DHHS has no doula program page. Retroactive claims may be possible once billing opens. The SPA requires a recommendation from a physician or other licensed practitioner. Doula services can be billed only once per pregnancy, with no extra pay for multiples. There's no fixed per-pregnancy total because visits are paid per 15 minutes with no cap per visit. DHHS's June 2026 notice lists the $12.50 and $500 rates. Most NH Medicaid members are in managed care plans; ask your plan whether it covers doulas yet. New Hampshire Medicaid has no doula enrollment or member page yet; the state licensing office (OPLC) page covers certification only.
