Visit structure and dates are confirmed by CMS-approved SPAs DE-24-0001 (approved 6/18/2024, effective 1/1/2024: 3 prenatal, 3 postpartum, birth attendance, 15-minute units with at most 6 units per visit) and DE-25-0002 (effective 4/2/2025: at least 1 prenatal visit, plus 5 more postpartum visits). Dollar rates come from Delaware First Health's provider page; DMMA's website has no doula page. The Delaware Certification Board's legacy pathway requires 15 clients. Delaware First Health says no authorization or referral is needed for the standard visits and adds 2 extra prenatal visits as a plan-specific value-added benefit (other plans may differ).
