The $1,621.78 total sometimes quoted adds the $100 incentive and the 5 extra postpartum visits (which need a provider's recommendation) to the $1,046.88 standard package. Visit structure and dates are confirmed by CMS-approved SPAs DE-24-0001 (approved 6/18/2024, effective 1/1/2024: 3 prenatal, 3 postpartum, birth attendance, 15-minute units with at most 6 units per visit) and DE-25-0002 (effective 4/2/2025: at least 1 prenatal visit, plus 5 more postpartum visits). Dollar rates come from Delaware First Health's provider page; DMMA's website has no doula page. The Delaware Certification Board's legacy pathway requires 15 clients. Delaware First Health says no authorization or referral is needed for the standard visits and adds 2 extra prenatal visits as a plan-specific value-added benefit (other plans may differ). Coverage runs from confirmed pregnancy through 180 days after delivery. Delaware Medicaid has no member page about doulas, so the member link goes to Delaware First Health's page.
