Rate cut: the maximum was $1,500 before July 1, 2026. HCPF's July 1, 2026 fee schedule lowers T1032 to $24.50 per 15 minutes and T1033 to $882, so the current maximum is $1,470. Limits are counted in minutes, not visits: 180 minutes prenatal and 180 minutes postpartum per member in a 12-month period, shared across all doulas a member sees. Benefit is available while pregnant and up to 365 days after birth. A written or electronic recommendation from an enrolled licensed provider (physician, PA, APN, midwife, psychologist, etc.) is required and the recommending NPI goes on the claim; no statewide standing recommendation was found. Doula claims go to the state fiscal agent (fee-for-service) even for members in physical-health managed care. Labor support must be in person. Member info: the Health First Colorado 'Benefits & Services' page has a Doula row (pregnancy up to 180 minutes, labor and delivery once per 12 months, postpartum up to 180 minutes, referral from OB or primary care provider). SPA CO-24-0004 was approved 8/19/2024 with an effective date of 7/1/2024.
