California details
The standard package is 1 initial visit plus 8 prenatal or postpartum visits in any mix, and 2 extended 3-hour postpartum visits after the pregnancy ends (11 visits total, plus labor support). DHCS's statewide standing recommendation (Nov 2023) covers this standard package, so no individual referral is needed; the extra 9 postpartum visits require a written recommendation from a licensed provider and cannot come from a standing order. Covered in fee-for-service and all Medi-Cal managed care plans; one visit per day. Services also cover miscarriage, stillbirth and abortion. The 2024 rate increase is funded through the MCO tax and Prop 35; DHCS's reimbursement FAQ (updated Sept 2025) says the rates remain in effect, and we found no 2026 change. Figures come from DHCS's All Plan Letter 23-024, its 2024 doula slides and the federally approved plan amendment (SPA 22-0002).
