Arizona details
There's no visit limit: AHCCCS sets no minimum or maximum number of visits, and the 2-hours-per-visit and once-per-9-months labor limits can be exceeded with prior authorization, so there's no fixed per-pregnancy total. All pregnant or postpartum AHCCCS members qualify (no high-risk requirement) during pregnancy and up to one year postpartum, but a referral from an AHCCCS-registered provider (physician, midwife, NP, PA, and several behavioral health and clinic provider types) is required; there is no statewide standing recommendation. Health plans may not require prior authorization for doula services. Doulas serving managed-care members must also credential and contract with each AHCCCS health plan. AHCCCS has no member page about doulas; ask your health plan.
