ForwardHealth's Online Handbook (policy through September 30, 2026) has no lactation consultation topic or code in the Physician, DME, FQHC, Community Health Center or Home Health service areas. Breastfeeding education appears only as a nutrition counseling topic within Prenatal Care Coordination (PNCC), which continues for 12 months postpartum only for members already receiving PNCC on the last day of pregnancy, and PNCC services are capped at $1,938.40 per pregnancy. KFF's 2021 state self-report listed lactation consultation as covered in all settings; the current handbook does not show a separate lactation benefit.
