Oklahoma details
SoonerCare covers services by IBCLCs who hold their own OHCA contract, for pregnant members and up to 12 months postpartum, in the IBCLC's office, the member's home, another outpatient setting or by telehealth (OAC 317:30-5-231 to 233, revised 09-01-2026); inpatient lactation services are paid inside the hospital per diem. The OHCA provider page lists S9443 (initial assessment) and S9445 (follow-up) and still says no referral is needed, but the 09-01-2026 rule text requires referral by a physician or other licensed provider.
