Michigan Medicaid and Healthy Michigan Plan pay for outpatient lactation support by an IBCLC, billed with S9443, separately from other same-day services. The IBCLC must be a Medicaid-enrolled physician, PA or APRN, or work under one who bills; MIHP agencies can bill for RN or social worker staff who hold the IBCLC. The October 2026 manual extends the window from 60 days (2016 bulletin) to 12 months post-delivery and allows simultaneous audio-visual visits; MOMS program beneficiaries are excluded.
