New benefit starting 4/1/2026 (SPA 25-0022, approved 12/19/2025). The state plan points to an online lookup tool for rates, so no total is given here. The 32-unit limit is one shared pool for prenatal and postpartum support, with no separate visit counts. Limits can be exceeded with prior authorization based on medical need. Telehealth is not allowed, and travel time and mileage are not covered. A Medicaid-enrolled qualified health professional must order doula services before they start, and the claim must include that provider's NPI. Limits apply per member, so seeing more than one doula does not add coverage. Utah Medicaid has no member page about doulas yet.
