There's no published total because the federally approved plan amendment (SPA 23-0014) ties doula pay to a percentage of the physician fee schedule. OHCA's published fee schedules list no doula (HD) rates, and the prenatal/postpartum code 59899 is 'MANUAL' priced at $0. The physician rate the labor payment is based on fell from $685.69 (Oct. 1, 2025) to $637.01 (July 1, 2026). Benefit: 8 prenatal/postpartum visits in any mix, each at least 60 minutes, plus 1 labor/delivery visit of any length. Prenatal/postpartum visits can be by telehealth, but labor support cannot, and there is no coverage at home births. A mom needs a SoonerCare Referral Form recommending doula care from an OB, CNM, physician, PA, APRN or licensed midwife. No prior authorization is needed for the standard 8 visits. Extra visits need PA and are allowed only for extenuating medical circumstances. Rule 317:30-5-1217 was revised 9/1/2026. SoonerCare Helpline: 1-800-987-7767.
