North Carolina details
NC Medicaid covers medical lactation services only when the breastfeeding infant has a chronic, episodic or acute condition (for example latch difficulty, prematurity, multiple birth, jaundice or poor weight gain); the claim is billed under the infant. IBCLCs must be employed or contracted by a physician practice, health department, FQHC/RHC or similar enrolled provider, which bills CPT 96156, 96158 or 96159 with the SC modifier. Group lactation services are not covered and WIC-eligible members are referred to WIC first. No prior approval is required.
