Oregon details
Visit structure: the global package has 2 prenatal and 2 postpartum visits, plus a separate pool of 4 extra visits usable before or after birth. No prior authorization is needed. Under ORS 414.668, access must not require a referral or another provider's signature. Postpartum visits are covered up to 12 months after pregnancy ends. Coverage dates from the original doula rule (OAR 410-130-0015), effective Jan. 1, 2014. Most OHP members are in CCOs, which must offer the same benefit but may have different billing rules. SB 1568 (2026, Chapter 92) sets minimum doula coverage (at least 12 visits or 24 hours plus labor and delivery) for CCO contracts and health plans starting or renewing in 2028 or later. It also requires OHA to review doula rates in even-numbered years, but no 2026 rate change was published as of today. OHA's legacy (experience) pathway requires 1 recommendation letter, 3 births and 3 postpartum visits, and 300 verifiable hours within 5 years. OHA has no member page about doulas; ask your CCO (coordinated care organization).
