Sources differ: one summary says Nevada raised the totals to $1,500 urban and $1,650 rural, but the federally approved plan amendment and the fee schedule show $1,300 and $1,430. Prior authorization is required for visits beyond 4 prenatal/postpartum + 1 labor. Services need a recommendation from a licensed practitioner. As of 5/27/2026 (MSM 603.4E), certification can come from the Nevada Certification Board or another state-approved training program. Doulas can serve members at home, office, hospital or freestanding birth center. Nevada Medicaid has no member page about doulas.
